A signed copy of the following documents must be turned in to your Head Coach or Team Parent prior to participating in practice.

• Player & Parent Code of Conduct/Contract
• Medical Release Form
  • A copy of a physical completed in the year 2025 must be included on the NCHSAA standard physical form
